Athletic clearance forms available for online submission

The process of completing annual athletic clearance forms just got easier thanks to a new service that allows parents to fill out and submit most of the forms on a computer.


Beginning this fall, parents of students in grades nine through 12 can choose between the traditional method of filling out the forms by hand or expediting the process through an online service. The new service is offered through an agreement between the district and Rank One Sports.


A student’s sports physical must still be filled out by a physician and submitted to the school in person before the first athletic practice. Each high school arranges specific dates and times for students to turn in athletic clearance forms to the school’s athletic coordinator.


Athletic coordinators will be able to call up the student’s grades on the computer, as well as any electronic forms parents have completed and signed online, said Rick Wells, director of athletics, health, and fitness.


They also will be able to quickly see at a glance if parents have paid athletic participation fees via a link available on the electronic forms website, he said. The fees will also continue to be accepted in person.


“This new online service will streamline the clearance process and hopefully create shorter wait times in line,” Wells said.


The new electronic athletic forms are available via a link on the school district website. On the Home page point to “Schools,” point to “Junior High” or “High School,” and then click the name of the school the student attends. Scroll down the page to the “Links” heading and click the “Electronic Forms” link.


The link will direct parents of students in grades nine through 12 to type in their student’s first and last name, student identification number, and school attending in 2015-16.

The computer program tracks when forms are complete or are awaiting completion.
