Filing a Declaration of Intent to Homeschool
A Declaration of Intent to provide Home-Based Instruction must be completed annually and returned to the Education Service Center (ESC), 302 - 2nd St. SE, Puyallup, WA 98372 or emailed to Shelly Teele (teelesa@puyallup.k12.wa.us). Declarations must be returned within two weeks of the beginning of any public school Individual Progress Report (IRP) period, trimester, or semester. If your child has previously attended a Puyallup public school, we ask that you withdraw your student upon filing your Declaration of Intent.
