Fall athletic clearance and concussion screening

In order to participate in junior high and high school athletics, students are required to complete the athletic clearance process.

Beginning in the 2016-17 school year Pu­­yallup School District will move to an online system to complete the forms required to participate in both junior high and high school sports. The online system is currently being used for 9-12 high school athletes and is new for junior high students this year.

The primary function of the RankOneSport web based service is to help organize, save time, improve communications, and provide documentation. It allows parents to complete eligibility paperwork and electronically sign permission forms, student handbook commitments, complete eligibility forms, etc. Such provisions expedite the athletic clearance process and serves as an electronic repository for athletic paperwork.

Students participating in high school or junior high sports this fall must compete ALL requirements below and attend one of the athletic clearance sessions scheduled during August at their school. Visit your school's webpage for the dates and times of these sessions.  

Junior High Sports Requirements

In order to participate in junior high sports, the following requirements must be met:
1. Complete the digital forms found at https://puyallupschools.rankonesport.com
  -  Cautions, Considerations and Responsibilities
  -  Steroids Flyer
  -  Athletic Eligibility Form
  -  Concussion / Sudden Cardiac Arrest Information Sheet
  -  Parent Permission

2. Electronic online forms are digitally signed by the parent and student, will be recorded as complete in the system database, and checked by the school Athletic Director when clearing athletes. There will be no need to bring these forms to the school for clearance. There is no log-in or account to remember. Parents will need to know the Student ID to complete the forms. The digitally signed forms are good for the 2016-17 school year and the 2017 summer activities. 
Ninth grade students should select the high school they are participating at, not their resident junior high school.

3. Submit a current
 physical examination form to the junior high school Athletic Director. Most physicals are good for two years. The physical must be valid for the entire sport season.

4. Submit a current baseline concussion testing form through ATI/Apple Physical Therapy if participating in football, soccer, wrestling or basketball. Information can be found on the PSD website under Programs / Activities & Athletics. Screening dates can be found at Concussion Screening for Fall 2016 Athletes.

5. Pay the Athletic Fee ($40 per season) and purchase a junior high school ASB Card ($30). These fees can be paid in person with the school bookkeeper prior to athletic clearance, or by using the EZ School Pay system at 
www.ezschoolpay.com (a convenience fee is charged). In either payment situation, bring your receipts with you to the Athletic Director during clearance.

6. Meet the minimum academic requirements during the previous semester
. Student must have passed a minimum 5 of their 6 classes and have a GPA of 2.0 or higher. Students not meeting these requirements will start the season on academic probation.

High School Sports Requirements

In order to participate at one of the three district high schools, the following requirements must be met:

  -  Concussion / Sudden Cardiac Arrest Information Sheet
  -  Parent Permission

2. Submit a current physical examination form to the high school Athletic Coordinator. Most physicals are good for two years. The physical must be valid for the entire sport season. (WIAA #17.11.0) D.C.

3. Submit a current baseline concussion testing form to the high school Athletic Coordinator. All Basketball, Football, Soccer, and Wrestling athletes must receive concussion baseline screening by/or under the supervision of an MD/DO/ARNP/PA-C/ATC who is trained in concussion management, prior to participation. 4. Submit a Clearance Card to the high school Athletic Coordinator. Students must submit a new Clearance Card every school year. These cards can be found in the Main Office at Rogers.

5. If a student has a life-threatening health condition or needs access to emergency medications, then an
Emergency Action Plan, signed by a physician, must also be submitted. This can be completed on the back of the Clearance Card or attached as a separate sheet.
